Serve as an advocate to all wards appointed by the State of Wisconsin Circuit Court to ensure appropriate services are being provided.
RESPONSIBILITIES:
Provide professional management of the ward's residence
Provision of education, health and general welfare
Consent to medical treatment and other personal, legal, financial and medical affairs
Advocate on behalf of wards to obtain necessary care
Visit and contact with wards to ensure appropriate services are being provided
Follow-up process for services
Perform assessments to determine eligibility for services
REQUIREMENTS:
Knowledge of local, state, and federal income support programs such as disability insurance benefits, Social Security, Supplemental Security Income, Food Share, Veterans' benefits, Medicare and Medical Assistance, and other supportive programs such as Long Term Care that could assist in supporting the ward is favorable
Superior oral and written communication
Ability to organize and work independently
Use analytical skills when detecting patterns, brainstorming, observing, interpreting data, and making decisions based on multiple factors and options available
Possess a valid driver's license, proof of vehicle insurance and reliable transportation
Bachelor's degree from a four-year college or university in Human Services or related field with a minimum of 2 years' experience is preferred or equivalent combination of education and experience
Knowledge in the provision of guardianship services through the Department of Health and Social Services Administrative Code DHS 85.and Wisconsin State Statutes Chapter 51 Mental Health Act, Chapter 54 Guardianship and Conservatorship and Chapter 55 Protective Service System considered favorable
